Pricing
Enrollment Fee: $125 one-time Monthly Plans:
● Essential Care (18-39): $135/month
● Comprehensive Care (40-59): $160/month
● Advanced Care (60+): $185/month
Important Note
Bioidentical Hormone Replacement Therapy (BHRT) and other advanced services are provided based on clinical need and are not included as part of the monthly membership. DOT physical exams and return-to-work evaluations are separate services with their own fee structure. Members receive a 10% discount on these services.
Hormone Therapy and Advanced Care
Bioidentical Hormone Therapy is offered based on individualized clinical evaluation and laboratory findings. Not all patients are candidates for hormone therapy. Risks, benefits, and alternatives are discussed prior to treatment.
Typical treatment intervals:
● Women: approximately every 3–4 months
● Men: approximately every 4–6 months
Treatment timing is individualized based on clinical response and laboratory evaluation.
